"Star Trek 1, The Motion Picture" includes the artist's name in the title, so when it plays it comes up:

Star Trek 1, The Motion Picture - Jerry Goldsmith - Jerry Goldsmith

I have a nitpicking question.

"Farewell Neverland" from the "Hook" soundtrack has a running time of approximately ten minutes. The version available at SST only plays the first two and a half minutes of the track. Is there a reason for this?

SAM tends to do this with some songs for some reason. I've tried changing the audio pipeling settings but that doesn't seem to help any. I will look into this further when I have more time.

You have Hana-Bi by Joe Hisaishi linked twice. It could be becasue you list the year of the CD to be 1996-1998. The CD was released in 1998.
